An HVAC service call intake checklist should collect enough verified context to route and prepare the visit without diagnosing equipment from a phone conversation. The core record covers the caller and service location, observed symptoms, equipment context, property access, occupancy or time constraints, prior work, reviewed safety boundaries, service-area and account rules, appointment state, and the exact handoff to dispatch or the technician.
Start with identity, location, and permission
- Caller name and preferred callback method.
- Service address, unit or suite, and location notes required to find the equipment.
- Relationship to the property: owner, tenant, manager, resident, commercial contact, or other approved role.
- Account or warranty locator only when the workflow authorizes it.
- Permission and contact requirements for tenant, landlord, property manager, or site representative.

Do not collect every possible field simply because the form allows it. Define which details are required to identify the job, make the handoff, and meet policy. Mark uncertain identity or authorization for review rather than converting the caller's statement into a verified account fact.

Keep emergency routing separate from HVAC priority
An HVAC priority such as no heat, no cooling, water near equipment, or commercial downtime is not automatically the same as an emergency requiring police, fire, or ambulance assistance. Conversely, a calm caller can report facts that cross an immediate-danger boundary. The business should have a locally and legally reviewed script that stops ordinary booking when that boundary triggers.

- Do not assure the caller that a reported condition is safe.
- Do not book an ordinary appointment in place of the approved emergency direction.
- Notify the internal owner according to the reviewed policy without representing the business as emergency services.
Build dispatch priority from reviewed factors
Priority can consider immediate-danger screening, vulnerable occupants or protected operations where the business has an approved policy, weather and loss-of-service impact, property-damage risk, commercial process impact, service agreement, prior commitment, job age, technician skill, parts or equipment needs, geography, and schedule capacity. The owner must approve the ordering. Do not present an illustrative class or response time as a universal industry rule.
- Emergency boundary: stop ordinary service flow and use the reviewed route.
- Qualified urgent review: dispatch lead evaluates time-sensitive impact and safe next action.

Confirm the appointment state precisely
Use distinct states: requested, offered, held, submitted, accepted by the scheduling destination, confirmed to the customer, changed, canceled, or failed. If the destination system times out, the state is uncertain—not confirmed. Record the window or appointment exactly, customer contact expectation, quoted fee only if sourced and authorized, access instructions, and anything the customer must do before arrival.
